Personal Body Information Sheet

This is a pretty self-explanatory and practical document. 

You can use this information sheet to (1) help students practice their Personal Body Information, (2) include in a personal portfolio and/or (3) include in a community folder that students can use while out in natural settings. 

In the context of this document, "Personal Body Information" consists of the following: 

• Name
• Height
• Weight
• Hair Color
• Eye Color
• Date of Birth
• Shoe Size
• Shirt Size

SO OFTEN, we see students with disabilities in high school who do not know or have access to this information. You can use this sheet in high school, for sure, but don't hesitate to use it (or something like it) early on as well! The sooner they learn this, the better prepared they will be for life as an adult.
